Every National Service personnel is required to complete and submit the NSS Evaluation Form to their District Office by the 15th of each month.
Failure to do so may result in the withholding of the personnel’s allowance.
Additionally, any form that is not signed and stamped by the personnel’s supervisor will be considered invalid.

To check if your Evaluation Form has been accepted by the NSS, follow these steps:
1. Log in to your NSS dashboard at https://portal.nss.gov.gh/sign-in using your credentials.
2. If your form has been accepted, it will disappear from the dashboard where you downloaded it. If it doesn’t, it means it hasn’t been approved.

3. If it has disappeared, Select the “YOUR ALLAWA” tab. (If you’re using a phone, tap the three dashes at the top-right corner to view this column.)
4. You will see a list of all your submitted Monthly Evaluation Forms, including the months, working days, number of days at post, date of submission, and status.
5. The status should be “SUBMITTED,” indicating that the NSS has approved the form.

By following these steps, you can easily check the status of your Evaluation Form and ensure that it has been properly submitted and accepted by the NSS.
